Root Canal Treatment: Keeping the Tooth You Already Have

When decay or injury reaches the nerve and blood supply inside a tooth, root canal treatment removes the affected tissue so the tooth can stay in place instead of being extracted. It is performed under local anesthesia and is intended to relieve pain, not cause it.
What to expect

How do I know if I need one?

Common signs include lingering pain with hot or cold, pain on biting, a persistent deep ache, a darkening tooth, or swelling near the gumline. Diagnosis requires an examination and X-ray — some teeth needing treatment have no symptoms at all.

What actually happens during the procedure?

The tooth is numbed, an opening is made, the inflamed or infected pulp tissue is removed, and the interior is cleaned, shaped, and sealed. Most patients describe the experience as similar to having a filling placed.

Why a crown usually follows

A tooth that has had root canal treatment has lost internal structure and becomes more brittle, particularly a back tooth that takes chewing load. A crown placed afterward protects it from fracture and is usually part of the treatment plan from the start.

What if I wait?

Untreated pulp infection does not resolve on its own. It can progress to an abscess, spread into surrounding bone, and eventually make the tooth unsalvageable. Swelling accompanied by fever or difficulty swallowing needs urgent attention — call the office immediately.

Why the Pulp Matters

The pulp is the soft tissue at the center of the tooth, containing the nerve and blood supply. Root canal treatment removes pulp tissue that has become inflamed or infected, then seals the space—relieving pain while keeping the surrounding tooth structure intact.

Cases referred to an endodontist

Retreatment of a previous root canal, teeth with unusually complex canal anatomy, and cases requiring surgical apicoectomy are referred to a specialist. Dr. Gill will tell you upfront which category your tooth falls into.

Common Questions

The procedure itself is done with local anesthesia and is aimed at relieving pain. Mild tenderness for a few days afterward is normal and usually managed with over-the-counter medication.
