Forms on this site are intended for general, non-clinical inquiries only — your name, contact details, whether you are a new or existing patient, and a broad reason for contact. Do not include symptoms, medical or dental history, medications, diagnoses, insurance identifiers, or images of a health condition. Clinical information should be provided through our secure patient forms, our patient portal, or by telephone.
When you submit a contact or appointment-request form, we receive the information you enter in it. Our hosting provider processes standard technical request information required to deliver the site securely. We use privacy-minimized analytics to understand aggregate traffic patterns.
We do not run advertising pixels, cross-site advertising trackers, or session-replay tools on this website. We do not sell personal information, and we do not use information submitted through this site to build advertising audiences.
Certain functions — appointment scheduling, patient forms, the patient portal, and online payment — are provided by our practice management platform and other vendors, and open in their own systems. Those services are governed by their own terms and by our agreements with them, including business associate agreements where protected health information is involved.
This site links to external platforms such as review sites, social media, and provider directories. We do not control those sites and are not responsible for their privacy practices.
Health information created or received by the dental practice in the course of your care is governed by federal and California health privacy law and by our Notice of Privacy Practices, which is available at the office and published on this site.
California residents have rights regarding personal information under California privacy law, which may include the right to know what personal information is collected, to request deletion, and to correct inaccurate information, subject to exceptions including information governed by health privacy law. This website is not directed at children, and we do not knowingly collect personal information from children through it. Information about minor patients is provided by a parent or guardian in the course of care.
