Your record

Requesting your records

You can ask for a copy of your dental record, including X-rays, for yourself or to send to another office. Call (559) 271-8400 or ask at the front desk; we will confirm your identity and, for transfers, where to send it. Your rights over that record are set out in our Notice of Privacy Practices.

Please do not email health details

Ordinary email and website forms are not secure. Send clinical information, photos of a problem, or insurance identifiers through the patient portal, or tell us over the phone.

Prescriptions and post-treatment questions

For questions after a procedure — a filling that feels high, a temporary crown that came loose, pain that is not settling — call the office. Do not wait for your next scheduled visit.

Monday to Friday, 9:00 am to 5:00 pm. Closed Saturday and Sunday.

Rescheduling
Call as early as you can. Advance notice lets us give the time to someone in pain and keeps your own care on track.
After hours
Leave a message and we return calls the next business day. For a medical emergency, call 911.
